Responsibilities and Duties

Our Homecare Professionals are compassionate, detail oriented, and enjoy working one on one with clients in their homes.

• Provide routine individualized care to the elderly, patients, or persons with disabilities
• Monitor and report changes in patient health status
• Work with a team to implement a comprehensive plan based on clients’ needs or illness
• Provide companionship and guidance to clients
• Documenting clients plans and on-going activities
• Monitor and report changes in clients symptoms or behavior
• Follow agency protocols to assure proper maintenance of client records, statistics, reports and records for purposes of evaluation and reporting of organization activities.

Qualifications and Skills:

• Experience in hospice, home health, or other related fields a plus
• High Degree of oral and written communication skills
• High ethical standards and character
• Self-motivated with the ability to work independently
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
