Responsibilities and Duties
Our Homecare Professionals are compassionate, detail oriented, and enjoy working one on one with clients in their homes.
- Provide routine individualized care to the elderly, patients, or persons with disabilities
- Monitor and report changes in patient health status
- Work with a team to implement a comprehensive plan based on clients’ needs or illness
- Provide companionship and guidance to clients
- Documenting clients plans and on-going activities
- Monitor and report changes in clients symptoms or behavior
- Follow agency protocols to assure proper maintenance of client records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
Qualifications and Skills:
- Experience in hospice, home health, or other related fields a plus
- High Degree of oral and written communication skills
- High ethical standards and character
- Self-motivated with the ability to work independently
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
