Care Manager Required Skills:
An experienced LPN with relevant Healthcare Experience. Minimum of 3 years experience in Home Healthcare or Case Management, specifically with the geriatric population.
Have a clear LPN license, be able to pass a Criminal Background Check and Drug Screen. Maintain a motor vehicle license and insurance per company policy.
An experienced RN with relevant Healthcare Experience. Minimum of 1 year experience in Home Healthcare or Case Management, specifically with the geriatric population.
Have a clear RN license, be able to pass a Criminal Background Check and Drug Screen. Maintain a motor vehicle license and insurance per company policy.
Highly proficient using computers, tablets, and/or smartphones.
Ability to work with minimal supervision.
A home computer with in-home internet.
Position Summary:
Responsible for providing in-home post-hospitalization and chronically ill care management assessments.
Facilitate the delivery of quality, caring, and cost-effective interventions.
Work with available resources both local/community based and through the insurance provider to achieve recovery and autonomy of the client.
Use their nursing knowledge and the nursing process to provide advocacy, assessment, planning, communication, education, resource management, and service coordination/implementation.
Perform other duties such as long-term care insurance assessments and medication review.
