Case Manager Job Opening

ReVIDA Recovery Centers is expanding the behavioral health staff in our Knoxville location. Our current need is for a Case Manager that possesses a heart of service, has a leadership mentality, and is eager to put the patient first and treat the patient with respect. Case Manager service Description: Integrates behavioral health into primary care and specialty medical settings through interdisciplinary care planning and monitoring patient progress and tracking patient outcomes. Supports conversations between physicians and behavioral health professionals to develop and monitor individualized treatment plans. Links patients to community resources including Narcotics Anonymous, Alcoholics Anonymous, peer supports, and other psychosocial resources to facilitate referrals and respond to social service needs. Tracks and supports patients when they obtain medical, behavioral health, or social services outside the practice. Required credentials: Bachelors degree in one of the following fields; Social Work, Psychology, Sociology, Human Services, or Psychiatric Rehabilitation OR Current certification in Tennessee as a Peer Recovery Specialist AND Has at least one year of substance abuse related clinical experience providing direct services to persons with a diagnosis of mental illness or substance abuse.
