We are seeking a Full-Time Social Worker to support the Cancer Center at Inova Fairfax Hospital.
The job duties for this position include:
- Perform intensive casework in an acute care setting for a medically complex population with primary psychiatric and/or addiction disorders.
- Coordinate and facilitate all aspects of discharge planning for assigned caseload in alignment with the program’s mission, values and vision.
- Work with multi-disciplinary team on implementing and evaluating patient care plans and progression throughout the continuum.
- Actively participate with treatment team to implement activities and processes that support the organizations strategic quality, cost and service objectives.
- Professional Development: Ensure improvements in practice settings by assuming responsibility for self-development.
- Actively participate in clinical performance improvement activities.
- Provide coordination of services and act as a key liaison between patient, patient’s family and the interdisciplinary healthcare members
Requirements
EXPERIENCE
1-2 years clinical and case management experience in an acute and post-acute health care environment.
EDUCATION
Master's in Social Work (MSW)
CERTIFICATION
CPR required
SPECIAL SKILLS
Working knowledge of case management processes needs assessment, principles of utilization review/quality assurance, discharge planning, and reimbursement structures (i.e. Medicare, Medicaid, and HMO).
Knowledge of regulatory requirements.
Advanced communication (oral and written) skills to effectively and professionally liaison with patients, families, and medical staff.
