What You Will Do:

Provides support services, under the supervision of the appropriate professional staff in accordance with state regulations, assists the client or family in the achievement of physical and emotional comfort according to the patient’s plan of care.

Essential Functions:
1. Provides personal care services per state/federal rules, regulations, and/or standards.
2. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning and doing household chores.
3. Plans and prepares nutritious meals, including shopping and assisting with feeding, as assigned.
4. Performs homemaking and other environmental services as assigned.
5. Observes, reports and documents any changes in patient’s status. If unable to perform a certain task, reports to the primary nurse.
6. Understands basic elements of body functioning and reports changes in patient’s body functions as indicated.
7. Recognizes emergency situations and implements appropriate emergency procedures per care center policy.
8. Follows care center procedures regarding infection control, handling of hazardous wastes, and safety measures.
9. Accurately documents care provided and submits notes and other required documents per care center policy.
10. Follows specific written instructions for personal care prepared by a registered nurse in accordance with the Plan of Care. Written instructions are to be received prior to the delivery of services.
11. Assists patient with prescribed exercises as instructed per nurse or therapist.
12. Provides respite for patient’s family/caregiver when on-site, as appropriate.
13. Performs and records simple urine tests for sugar, acetane, and albumin.
14. Attends funerals/call hours when appropriate as assigned by the bereavement coordinator.
15. Performs other duties as assigned.

Qualifications

Required:

Knowledge, Skills and Abilities:
  • Ability to work independently with minimal direct supervision.
  • Ability to work as a team member.
  • Familiarity with general use and functions of the computer, such as usernames and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, windows, and use of navigation buttons and tool bars.
  • Ability to self-manage online HR services and online training programs.

Qualifications:
Required-
1. High school diploma or equivalent.
2. Six (6+) months full-time direct, patient care experience within the last five (5) years, in an institution or care center setting.
3. Meets federal/state requirements for this position. Home Health Aide (HHA) competence will conform with requirements in the Medicare Conditions of Participation found in 42CFR 484.36. Must be able to successfully pass the competency program of the care center.

4. Upon hire, HHAs will have received at least 75-hours of training, the content of which addresses all subjects listed at 484.80(b)(3).
5. Has not had a consecutive twenty-four (24) month or more lapse in practice for compensation since last competency training.
6. Current CPR certification.

Preferred-
1. Current state certification (or registration) as a nurse assistant (and as per state certification/ registration requirements).
2. Minimum of one (1) year full-time direct, patient care experience within the last five (5) years, in an institution or agency setting.

Working Conditions:
  • Frequent degree of travel. In instances where employee performs tasks that require the use of his/her automobile, evidence of current driver’s license and agency required liability insurance is required.
  • Frequently required to perform simple motor skills and manipulative skills such as sitting, crawling, climbing, and kneeling.
  • Frequent exposure to occupational hazards including, but not limited to, heat, cold, varying weather conditions, noise, vibration, fumes, dust, radiation and chemotherapeutics.
  • Frequently required to reach above shoulder level.
  • Frequent exposure to toxic chemicals.
  • Continually required to perform gross body coordination such as standing, walking, bending/stooping, squatting while performing tasks and balancing, lifting, carrying, pushing or pulling objects.
  • Continually required to see, hear, speak, and use one or both hands.
  • Continuous exposure to potentially infectious waste and to body fluids.
