Job Purpose:

The Chaplain/Bereavement Coordinator helps provide spiritual support to Hospice patients, families, caregivers, staff, and volunteers. The Chaplain/Bereavement Coordinator will help maintain an organized bereavement program that will support families/caregivers of Hospice patients during their time of grief up to 13 months. The ideal candidate will exhibit tolerance, compassion, and tact and is able to meet people where they are and provide supportive care in a variety of settings and with all people. The candidate must be a good listener and communicator both one-on-one, with patients and care-givers, and in team settings. This position reports to the Senior Chaplain/Bereavement Coordinator.

Education:

  • Graduate of an accredited school of theology, seminary, or Clinical Pastoral Education Certification.
  • Prefer at least 1 semester of Clinical Pastoral Educations Units or current enrollment in CPE.
  • Licensure and/or ordination as a minister preferred.
  • Training in grief and bereavement counseling preferred.

Experience:

Five years experience in pastoral care work related to the dying process and bereavement.

  • Responsibilities:
  • Assists patients, caregivers, families with spiritual care related to death and dying.
  • Visits patients and completes a spiritual assessment, documents via electronic medical device, and reports to the interdisciplinary team.
  • Acts as a liaison between patient, family, local clergy and interdisciplinary team.
  • Participates in the interdisciplinary team and provides spiritual input into the plan of care.
  • Helps interpret spiritual issues for the interdisciplinary team.
  • Seeks Hospice support from clergy of various denominations.
  • Provides bereavement services to the families and caregivers of Hospice patients.
  • Evaluates each incident of bereaved on a case by case basis and develops a bereavement plan of care in collaboration with the interdisciplinary team.
  • Helps establish and maintain a bereavement file on each bereaved caregiver.
  • Helps organize a bi-annual Memorial Service for bereaved families/caregivers.
  • Assists with volunteer training involving bereavement services.
  • Serves as a resource person in terms of community support groups available.
  • Networks with community resources and represents Hospice in the community
  • Participates in staff development and in-service programs.
  • Represents the Hospice Agency at meetings and workshops when appropriate.
  • Demonstrates integrity and maturity in handling the confidential aspects of this position.
  • Provides excellent internal and external customer service.

  • Strength: Position requires ability to push/pull objects of 20 pounds and the ability to lift objects more than 50 pounds.
  • Manual Dexterity: Position requires incumbent to constantly perform moderately difficult manipulative skills, such as driving a car, visiting with patients & typing.
  • Coordination: Position requires incumbent to perform gross body coordination, such as walking, visiting with patients and basic computer skills.
  • Visual Discrimination: Position requires incumbent to see objects closely, as in reading and discriminate colors in electrical wires or warning lights.
  • Hearing: This position requires ability to hear normal sounds with some background noise.Mental Requirements:

  • Attention Span: This position requires the ability to constantly attend to tasks or functions as required to complete the tasks.
  • Communications: Position requires the ability to communicate verbally and to communicate in written word using advanced business writing skills.Working and Environmental Conditions: Employee may occasionally be exposed to unpleasant, unsanitary conditions in the patient home. There may also be exposure to stressful family dynamics in the patient’s home. Road conditions and weather may be hazardous at times.
