Facility Social Worker-60 West Secure Care Options

Our eleven skilled nursing home facilities, as well as our Homecare and Touchpoints Therapy entities are located in the Greater Hartford area.

We’re looking for a full-time, Licensed-Facility Social Worker at our 60 West-Secure Care Optionslocation. The Social Worker will work with residents in the facility by identifying their psycho-social, mental and emotional needs and providing professional social work services in accordance with applicable regulations. Hours may require occasional work outside of standard office hours, including some weekends.

Our Social Workers are responsible for:

• Providing medically related social services to residents of the facility as well as family members as needed.
• Conducting face to face resident interviews and assessments of resident cognitive, mood, behavioral status, in addition to other psycho-social factors.
• Participating in planning for resident discharges.

Candidates must have:

• A minimum of a Bachelor's Degree in Social Work from a CSWE accredited undergraduate/graduate degree program is required for this position, as per CT state regulations.
• At least 1 year of long term healthcare experience preferred.
• Understands and meets all government requirements of social service documentation.
• Works with the interdisciplinary team and administration to promote and protect resident rights and psychological well-being.
