The Care Coordinator coordinates services for Medicaid recipients in order to improve the management of chronic diseases. This position is responsible for health education, counseling, follow-up appointments, and resolving transportation issues. These services are provided via phone, in person, and/or in the home via local daytime travel through a reliable means of transportation insured in accordance with Company policy.

REQUIRED:
- Bachelor’s Degree in Social Work or Nursing, or equivalent experience
- Current RN or SW License in good standing with the appropriate State of Alabama Board
- Valid driver’s license in good standing
- Excellent interview and telephone skills, as well as the ability to deal with recipients in a caring and helpful (assertive and tactful) manner
- Working knowledge of health related service delivery systems
- Excellent communication and relationship skills
- Ability to analyze varied environmental factors in relation to members’ well being
- Ability to locate, augment and develop resources, including information on services offered by other agencies
- Organization and time management skills
- Ability to be flexible, adaptable, and to work effectively in a variety of settings
- Basic computer skills
- Master’s Degree in Social Work
- Certified Case Manager (CCM)
- Less than 1 years’ experience in case management, human services, public health, or experience with the under insured population
- Ability to utilize Microsoft Word and Excel
