Responsibilities:

• Assist Families in navigation and accessing services through developing a working partnership with provider agencies
• Provide local resource information to any family requesting assistance
• Link families to local Support and Information groups or help develop groups if and when no other resources are available
• Work with families and youth who have complex needs to build a plan that incorporates both formal supports and informal supports

Qualifications:

• 1+ years of experience working with mental health disorders
• Ability to communicate effectively both on the phone, in person, and in email
• Flexibility with scheduling for clients needs
• Ability to maintain strict confidentiality

• Associates or better
• High School or better

• Some college or better in Social Work or related field
