Case Manager - Community Based Case Management - Mental Health Job Opening

Community Based Case Managers Provide specialized case management services for members diagnosed with serious and persistent mental illness. Responsibilities include, but are not limited to; completing psychosocial assessments, needs assessments, creating recovery plans, providing education and support for members and others in support network, linking members to community resources to further independence and stability and working collaboratively with fellow team members.

Case managers assist consumers one on one in the community. You must be comfortable working independently in the community with consumers and assisting them to various appointments including; housing screenings, court, Quest/Social security appointments, psychiatrist/psychologist appointments etc...

Education: Preferred BA or MA in psychology, sociology, counseling or related behavioral health field. Minimum BA and 12 credit hours in psych or psych related.

Experience required: 1.5 years of specialized experience working with in the mental health field.

Must have valid drivers license

Must have a Clean Abstract - no more than 2 moving violations in the past 3 years. No Distracted Driving (texting/phone) or DUI/DWI in past 3 years.
