Job Description

POSITION SUMMARY:

Responsible for the care-taking of Country Place Senior Living Residents according to Company Policy and applicable state regulatory requirements and standards. Remains pro-active in provision of care and relationships with Residents. Promptly reports changes, pertinent issues, safety hazards, etc., to Administrator, Resident’s family and/or Resident’s physician or medical Administrator. Maintains professional courtesy and demeanor with Residents, families, regulatory officials, staff and visitors at all times.

KEY AREAS OF RESPONSIBILITY:

  • Implements and operates within the policies, procedures and guidelines of Country Place Senior Living.
  • Conducts, in cooperation with the Administrator, the initial and ongoing assessments of each Resident, utilizing the nursing process and appropriate forms (FCS, NSA, HCP) to determine Resident need.
  • In cooperation with the Administrator, develops, implements, and updates Negotiated Service Agreement/Health Care Plan for each Resident.
  • Administers or supervises the administration of medications and treatments to Residents by checking the MAR and reviewing the medication administration practices of each CMA/LPN periodically.
  • Practices the standard precautions of infection control, and monitors staff and Residents for compliance to local/state health code/infection control practices.
  • Carries out and/or supervises the implementation of physician’s orders. Also documents refusal to follow physician's orders and monitors situation.
  • After consultation with Administrator, coordinates all medical services provided by Country Place Senior Living including, but not limited to: pharmacy, home health, services and therapies.
  • Documents and maintains records of all Resident procedures, instructions, observations and progress per documentation guidelines and policy & procedures.
  • Completes documentation in a timely manner, such as doctor appointment, new orders received, nursing notes, incident reports, etc.
  • Initiates and maintains an on call schedule 24 hours a day when not in resident by carrying a phone.
  • Communicates Resident information to the resident staff as indicated, through direct communication, staff communication, books, and/or staff meetings/in-services.
  • Communicates with on-call nurses and documents all procedures, instructions, observations, etc.
  • Reports changes in the condition of the Resident or the family situation to the Administrator, health care provider, staff and/or family as indicated.
  • Coordinates Resident services and needs; communicates, in cooperation with the Administrator, changes in service to direct care staff in a timely manner, and initiates changes on Functional Capacity Screen and Negotiated Service Agreement/Health Care Plan.
  • Participates in the orientation of staff members, in-service programs, and medical staff evaluation, all to be done in communication with the Administrator.
  • Assists the Administrator in the selection of medical staff, ordering of medical supplies, the assessment of the facility's ability to meet each Resident’s needs on an on-going basis while adhering to staffing and nursing budget guidelines.
  • Assures all staff/Residents have appropriate health screening/physicals (as required by state law) and TB test.
  • Provides direct Resident care as staffing and/or Resident need indicates.
  • Performs others duties as assigned by the Administrator.
  • Works scheduled shift, scheduled weekends, holidays, and rotating shifts (works the floor) in absence of sufficient, qualified staff (e.g.: employee ill, no call/no show, short of staff).
  • Monitors Resident’s supplies and provides a list of needs to the family and/or the Administrator.
  • Monitors, reports and records Resident incontinence. Assists with toileting.
  • Assists Residents with instrumental activities of daily as indicated by Resident’s needs and the policies of Country Place Senior Living.
  • Practices the standard precautions of infection control at all times.
  • Adheres to procedures for fire, life safety, disaster, work safety and other emergency procedures
  • Provides timely and accurate reporting.
  • Continually demonstrates the values of Country Place Senior Living.
  • Performs other duties as may be assigned by the Administrator.

POSITION REQUIREMENTS

  • Associates Degree preferred; High School graduate or GED required.
  • Graduate of an accredited school of nursing, with a current state license, and clinical experience with at least six months working with the elderly.
  • 2+ years supervisory experience.
  • Positive demeanor, high energy level and ability to move smoothly between shifting priorities.
  • Verbal and written communication skills in the English language necessary to communicate and work effectively with various levels of staff, Residents, family members and the public, and for required written documentation or reports.
  • Ability to speak, read and write English.

  • Fulfills work schedule which may include weekends, holidays, rotating shifts and overtime.
  • Maintain continual awareness of potential hazards, taking necessary measures to ensure Resident safety.
  • Aptitude and ability to relate to personality styles, temperaments and needs of individual Residents by adjusting communication, disposition, demeanor, etc., accordingly.
  • Demonstrates a warm, caring and outgoing personality.

PHYSICAL REQUIRMENTS

  • Ability to sit, stand or walk for long periods of time. Full range of motion capability; arms and trunk. Ability to stoop, kneel, bend, twist, reach and climb.
  • Gross and fine motor coordination to allow the manipulation of equipment utilized in the performance of activities.
  • Ability to lift/carry medium weight (up to 40 lbs.), light pushing/pulling (up to 60 lbs.).
  • Ability to provide personal care to Residents which includes ambulating, lifting, toileting and turning the individual of at least 150 lbs.
  • Ability to hear within normal ranges, vision (corrected if necessary) to allow reading and the discernment of color, and normal sense of touch.
  • Ability to withstand temperature fluctuations normally found within an office environment, and varying weather conditions which may exist while performing outdoor duties such as supporting activities and events.

On-Call Hours are Friday (6:00 pm) to Monday (6:00 am).

$50 On Call Pay and hourly pay if called in (minimum of 2 hours)
