Responsibilities may include:
- Conducting a variety of home visits with patients including start of care, routine, re-certification and resumption of care.
- Providing education to patients and their families.
- Utilizing our electronic medical record system, Brightree, to complete documentation including OASIS.
- Maintaining current patient records and progress notes.
- Developing and establishing a plan of care with physician.
- Supervising LPNs and Home Health Aides and document delivery of care as required.
Requirements for this position include:
- Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program.
- RN license.
- Nursing experience preferably within Home Health specialty.
- Valid driver's license, car insurance and reliable transportation.
- Willingness to travel throughout assigned service area.
