Responsibilities may include:
  • Conducting a variety of home visits with patients including start of care, routine, re-certification and resumption of care.
  • Providing education to patients and their families.
  • Utilizing our electronic medical record system, Brightree, to complete documentation including OASIS.
  • Maintaining current patient records and progress notes.
  • Developing and establishing a plan of care with physician.
  • Supervising LPNs and Home Health Aides and document delivery of care as required.

Requirements for this position include:
  • Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program.
  • RN license.
  • Nursing experience preferably within Home Health specialty.
  • Valid driver's license, car insurance and reliable transportation.
  • Willingness to travel throughout assigned service area.
