CARES Case Manager Job Opening

The CARES program is a case management program for families referred by local Departments of Social Services, that may have barriers to finding employment based on substance use disorders and/or mental health needs. Case managers provide services and additional referrals for services based on individual and family needs. Case managers also have close collaboration with local Department of Social Services to ensure that medical exemptions, SSI applications or work requirements are completed as necessary. This position will assist in providing individualized services for consumers identified as eligible for Case Management services due to serious mental illness, substance use disorder, and/or developmental disability. Position provides help in coordinating services and resources needed by the consumer to maintain and/or return to community living as well as direct services to clients. Cell phone, laptop, weekly supervision and mileage reimbursement provided.

Graduation from an accredited college or university with a Bachelor’s degree in Social Work or human services related field with at least one year of experience in a setting working with children or adults with needs in the areas of mental health, substance use, or developmental disability preferred. An appropriate applicant will demonstrate knowledge of case management/social work practices and resources available to help consumers in the community. Valid Virginia driver’s license with a safe driving record required.
