DD Support Coordinator/Case Manager I or II Job Opening

Provides supports services to individuals with Developmental Disabilities receiving Targeted Case Management/Support Coordination services. Works as part of a case management/support coordination team to ensure Medicaid, Licensure and Department of Justice standards are met. Coordinates and writes Person Centered Plans, Plans for Support, makes referrals as necessary and coordinates supports with other providers. This position will primarily work adults. Occasional evening work is required.

Case Manager I: Bachelor's Degree in special education, social work, psychology or other acceptable human services related field or an unrelated Bachelor’s Degree that includes at least 15 semester credits in a human services field. Clinical experience working with individuals with mental health diagnoses is helpful.

Case Manager II: Bachelor's Degree in special education, social work, psychology or other acceptable human services related field with at least one year of applicable related clinical experience working with individuals with developmental disabilities and/or behavior health diagnoses or an unrelated Bachelor’s Degree that includes at least 15 semester credits in a human services field and at least three years of applicable clinical experience. Virginia DMAS’ QDDP status is required. QMHP status is helpful.

Either position requires: CPR and FA (training provided), possession of a valid driver’s license (Virginia, unless otherwise approved by agency) and an acceptable driving record must be maintained throughout employment.
