iCare is currently looking for a Social Worker at Touchpoints at Farmington located in Farmington, CT.

Responsibilities and Duties

• Provide medically related social services to residents of the facility as well as family members as needed
• Conduct face to face resident interviews and assessments of resident cognitive, mood, behavioral status, in addition to other psycho-social factors
• Participate in planning for resident discharges

Qualifications and Skills

• Must have at least 1 year of long term healthcare experience
• Understands and meets all government requirements of social service documentation
• Works with the interdisciplinary team and administration to promote and protect resident rights and psychological well being
