Responsible for providing caregiver education and support that will enable the caregiver to be the first and foremost teacher for their child(ren) in their home environment.
Essential Duties
· Identify, recruit and enroll homebound families to participate in the program;
· Conduct initial home visit and assess the needs of program participants;
· Plan and develop with the caregiver an individualized program using educational activity plans based on their child's assessment and incorporating the family's strengths and needs;
· Conduct and provide: early screening and developmental checklists; testing, referrals and in-home activities;
· Obtain and maintain records of families and track their progress.
Other Duties
Other duties as assigned including, but not limited to, completing all forms and documentation such as reports related to grant objectives, professional development evaluations, and daily logs.
Work Hours
· 7:00 a.m. to 3:30 p.m. Monday through Friday or as otherwise determined by the supervisor;
· Position requires regular and predictable attendance;
· Some events on nights or weekends at various times depending on the event.
Working Conditions
· Home or Office setting;
· Travel in personal vehicle to and from home visits;
· Travel to the neighbor islands and the continental U.S. may be required.
Equipment Use
May include use of: program and/or personal vehicle, platform dolly, hand trucks, computer, laptop, laminator, Cricut, copier, shredder, fax machine, video and camera equipment, generator, speaker system, iPod, office elevator, warehouse gate, nametag maker, ice machine, stove, oven, refrigerator, microwave, paper cutter, office phones, work cell phones/Bluetooth.
Mental, Physical and Communication Demands
Mental:
· Consistent adherence to the practice of Partners in Development Foundation’s (PIDF) five core values that are used as a guiding force in the success of all of its programs;
· Must be keenly aware of and attentive to program participants;
· Must analyze and problem solve issues;
· Must be able to prioritize and focus on multiple tasks that require analysis and attention to detail;
· Must possess and exercise strong interpersonal and organizational skills;
· Must be able to cope with stress that may be induced by the situations of our clients.
Physical:
· Requires lifting and carrying up to 40 pounds on a regular basis;
· Must be able to sit and work on floor; kneel and bend.
Communication:
· Must exhibit courteous and professional demeanor towards program participants, vendors and partners, staff and administration via verbal, written, physical, and electronic correspondence;
· Requires good oral and written communication skills;
· Must be able to handle and maintain highly confidential information.
Minimum Qualification Requirements
· Bachelor’s degree in social work or human services related field;
· At least two (2) years working with families in crisis; at least one (1) year of experience in a social service or education related field; at least one (1) year of home visiting experience;
· Three years or more of experience in home visiting; experience in and knowledge of child development
· Proficient in Microsoft Office including but not limited to Word, Excel and PowerPoint;
· Must have access to a vehicle, have a valid driver's license, a safe and responsible driving record, and current no-fault auto insurance, with at least $100k in bodily collision per person and $25k in property damage liability (operating personal vehicle for work purposes).
