General Description:

The Unit Nurse Manager of the Skilled Nursing Unit (Northern Pines, Southern Pines and Post Acute West Units) is accountable for the planning, organizing, implementing and directing of resident care to assure quality resident care in accordance with current applicable Federal, State, and standards, guidelines and regulations.

Essential Job Functions:

1. Serves as a role model for all staff.

2. Promotes quality resident care through respect, caring and teamwork.

3. Supervises orientation of new staff.

4. Conducts monthly staff meetings to identify needs, encourage staff input in problem solving to assure that unit, resident and staff needs are met.

5. Conducts in-services in a timely manner to facilitate continuous quality improvement.

6. Establishes positive working relationships with residents, resident’s families and problem solve to meet resident needs.

7. Demonstrates knowledge of facility policies and procedures.

8. Supports staffing needs of unit.

9. Supports Administrative decisions in a positive manner.

10. Attends continuing education programs.

11. Attends Management Meetings.

12. Serves as resource to all staff.

13. Coordinates and attends Resident Care Plan meetings with families, residents, staff (including C.N.A.s), and Interdisciplinary Team.

14. Maintains positive working relationship with physicians.

15. Schedules and manages physician visits.

16. Organizes communications/telephone calls with physicians.

17. Supervises documentation to ensure compliance with State, Federal, and Local guidelines.

18. Collaborates with Supervisors to assure continuity of care across continuum.

19. Reports to Director of Nursing Services, communicating issues on the Skilled Nursing Unit in a timely manner.

20. Implements staff recognition programs.

21. Assists with transcription, documentation, admissions as appropriate.

22. Educates staff and initiate new resident care record forms.

Work Area and Working Conditions:

Entire Health Center facility

Entry Level Qualifications:

1. Graduate of accredited School of Nursing or BSN program.

2. Current registration in the State of Connecticut.

3. Must have Long Term Care experience. Experience in Geriatrics, including one year in a supervisory capacity.

4. Training in rehabilitative and restorative nursing practice.

5. Certification in Gerontology is a plus.

6. Must have tolerance and compassion for the geriatric population.

7. Must be a team leader and a team player. Able to work with others.

8. Must be knowledgeable of infection control policies and procedures and universal precautions.

Key Accountabilities:

1. Plans, organizes and carries out supervision of all services on unit and initiates corrective action as necessary.

2. Demonstrates excellent rapport with staff. Demonstrates knowledge and understanding of Medicare and Managed Care regulation.

3. Educates staff and communicates with other disciplines on process and requirements.

4. Ability to evaluate staff, situations, relationships in an effective, constructive, and positive manner.

5. Willingness to and efficiently render charge nurse’s duties and responsibilities in the event staffing is needed.

6. Manages and coordinates the nursing unit in order to provide quality resident care and effective/efficient utilization of resources.

7. Serves as a resource person for staff nurses and C.N.A.s regarding resident care.

8. Develops goals and objectives for the clinical unit and establishes an action plan to achieve the goals.

9. Plans, schedules and coordinates timely care plan meetings with resident, family and the interdisciplinary team.

10. Supervises documentation in the medical record to ensure compliance with State, and Federal regulations.

12. Collaborates with off staff and supervisors to coordinate and maintain consistency of resident care across 24 hours.

13. Participates in the Quality Improvement Program and involves staff as appropriate.

14. Keeps Director of Nursing Services informed of issues and status of the Skilled Nursing Unit.

15. Is responsible for the initiation and completion of all MDS and Resident Care Plans (RCP) within required time frame on all North, South, and West Wing residents. In coordination with the PPS Coordinator, coordinates completion of the MDS and RCP by the Interdisciplinary Team.

16. Utilizes supervising skills to coach, counsel, develop, empower, recognize and manage the performance of all employees under their direction. Actively promotes quality, customer satisfaction and continuous improvement in daily activities.
