• Alexander City, AlabamaSummary: Responsible for the supervision and safety of individuals. Responsible for overseeing and coordinating implementation of treatment/service plans, including behavior management and contingency programs. Provides instruction in Basic Living Skills and Family Support and Education as outlined in the individual's treatment plan and under the direction of clinical staff.
Individual Care: Supervises individual activities, including completion of daily chores, outings, training, grooming, bathing, toileting, meal-time activities. Ensures individual safety. Follows a schedule of activities. Monitors individual physical and mental health, ensuring that individual needs are met and maintained. Assists individuals as needed, to include lifting and other physical intervention as needed. May administer individual medication as prescribed by a physician and accompany individuals to medical appointments. Reports and documents incidents and/to unusual problems to supervisory or on-call staff. Models appropriate social skills. Ensures individual rights are observed and protected. Ensures individual clothing needs are met and that consumer is provided a suitable living environment. Completes program chores as assigned; i.e. sweeping, dusting, cooking, shopping, washing and drying individual clothing, changing linen. Follows menus approved by dietitian. Assists with menu planning (when making substitutions to menus ensure approval by dietitian), following menus, and preparation of meals. Ensures that meals are served family style and that appropriate table manners are modeled by staff during meal time. Ensures compliance with special diets. May schedule individual appointments for needed services (medical, dental, etc.). Responsible for reporting refusal of medications, medication side effects, or any problems with medication administration to nursing staff.
Program Services: Assists in implementation of the individual's treatment, habilitation, and/or service plans. Assists with implementation of the program’s contingency program (i.e., level system, token economy, etc.). Responsible for supervising program services in absence of the program director or therapist. Leads groups as assigned. Implements behavior management procedures as assigned and/or as indicated by a contingency program or the individual’s behavior plan. Contacts individual relatives or other significant others as appropriate. Completes fire drills, severe weather drills, and nightly bed checks. Reviews individual records to ensure familiarity with each individual's social and medical history.
Case Management Services: May be certified as a case manager and assigned a caseload. Provides crisis intervention as appropriate for individuals. Assures compliance with targeted case management standards.
Bachelor’s degree, in a human services fields of study. Valid driver’s license and safe driving record. Must meet conditional employment requirements.
