The Fairfield County Community Counseling department is seeking a passionate and excited individual to provide community-based case management services to a diverse clientele of children and families with mental health diagnoses in the city of Lancaster and Fairfield County. The ideal candidate will have a genuine interest in providing individual and family support, skills, advocacy, and crisis management in a home-based setting. Supervised OhioGuidestone workers have excellent communication skills and establish relationships, collaborating with the treatment team and community providers. Candidates with experience in Alcohol and Drug, and/or Co-occurring disorders are welcomed to apply!
Essential functions:
Develop and maintain a service relationship with youth and family
Consult with an ODMH Mental Health Assessment, develops a service/treatment plan
Provides Ohio Department of Mental Health and Addiction Service, Community Psychiatric Supportive Treatment Service (CPST) or AoD Case Management
Provides parents with information on disciplining, behavior management, household management and communication and relationship enhancement when needed; teaches youth social skills
Provides crisis intervention and mediation as necessary
Employees maintain a productivity of 52% for services
If Mentoring services are provided, employee will maintain a productivity of 85%
Coordinates, makes referrals, or arranges for appropriate community resources when needed, acts as an advocate if necessary
Teaches family how to utilize the systems
Ensures that children are in the proper educational setting
Maintains ongoing contacts (phone, meetings, conferences) with other professionals involved, including collateral contacts with community resources/services
Attends all court hearings
Organization and maintenance of documentation of written client information and case notes
Record keeping and copying of necessary information, dissemination to appropriate individuals within the agency and any outside agencies based on departmental requirements
Coordinating and providing service and support to their assigned clients and families or care givers in the school environment
Provide school personnel and parents with information on disciplining, behavior management, household management, communication and relationship enhancement
Consult with school personnel and participate in team meetings as necessary
Meet with school personnel to establish parameters of school based mental health services and meet with school personnel on an as needed basis or as requested throughout the school year
Inform appropriate school personnel of clients being served maintaining confidentiality
Educate school personnel on various mental health conditions/diagnoses and assist with effective classroom intervention/strategies
Performance/Physical Requirements:
Work is performed in the homes of clients and several community agencies. This involves extensive driving.
English reading and writing skills required
Interacts verbally with coworkers, representatives from outside agencies, and families
Attention to detail and confidentiality required
Works flexible hours and is on pager system
Requires typing and computer competency, including Windows applications for moderate levels of typing case notes and other paperwork.
Qualifications:
An Associate’s degree in Social Work*, Counseling or a related field required
A Bachelor’s degree is preferred
Must have a valid Ohio Driver’s License, proof of auto insurance and safe driving record
- If exclusively providing AoD services, candidate may be employed with proof of High School Diploma or GED and proof of CDCA license or higher Chemical Dependency credential.
