Job Description:
The Up to 3 Early Intervention program in the Center for Persons with Disabilities at Utah State University and is an in home pediatric program in northern Utah which strives to provide quality, evidence-based therapy services in a nurturing & supportive natural environment.
We are looking for a dynamic, creative and committed Physical Therapist to join our interdisciplinary therapy team comprised of speech-language pathologists, occupational therapists, physical therapists, behavior analysts, child development specialists, behavior technicians, vision and hearing therapists as well as autism specialists. This position entails a flexible work schedule, providing individual services to children age birth to 3 years of age with varying abilities.
Responsibilities
Duties:
- Administer evaluations and assessments to help determine eligibility
- Develop individualized service plans as part of a child’s team
- Provide 1:1 therapy in the natural environment (often a the child’s home)
- Provide parent education and training
- Collaborate with other disciplines as needed/requested to ensure quality of care
- Conduct assessments (initial, on-going, and discharge) in conjunction with appropriate team members
- Adhere to university travel and driving policies
Qualifications
Minimum Requirements:
- Must have valid Utah Physical Therapy license.
- Must be able to pass a background check.
- Maintenance of appropriate professional affiliation(s) needed to supervise undergraduate and graduate students during practicum, student teaching, and internships.
- Excellent written and verbal communication skills
- Must have a reliable car, proof of auto insurance and a valid driver's license.
- Proof of immunization records and TB required.
- CPR certification is required.

Preferred Requirements:
- Previous pediatric treatment experience
- Multilingual ability
- Baby Watch Early Intervention Credential
Physical Requirements:
- Therapy activities may require playing on the floor or settings where the furniture is child size
- Careful attention to the safety of the client and surroundings is required
- Sudden and quick movements may be required
- Walking, sitting, kneeling, stooping, crouching, crawling, twisting, reaching above head, bending and lifting up to 25 pounds
- Sustained physical activity may be required for up to 60 minutes
