• Manchester, ConnecticutResponsible for coordinating all aspects of clinical treatment for assigned clients with mental health issues and/or serious emotional disorders. Coordinates all aspects of care with therapeutic providers. Coordinates matching efforts between the TFC Supervisor and Recruiter to meet the needs of the youth. Maintains contact with and consults with relevant collaborative parties. Provides support and consultation to case managers and foster parents. Shares on call responsibilities with the TFC Supervisor. Participates as full member of the clinical team and the program and is responsible for maintaining client records.

DUTIES & RESPONSIBILITIES:
- Provides assessment and treatment interventions consistent with fidelity to evidence based model if applicable.
- Documents ongoing client progress and outcomes, including but not limited to all biopsychosocial assessments, ICMPs, and treatment plans.
- Demonstrates competency in age related or specialty issues and developmental needs for each population served.
- Works with clients to develop appropriate discharge plans utilizing natural supports.
- Maintains collaborative relationships with family members and other service providers
- Attends PPT meetings, ACR and other DCF treatment planning/other provider meetings to support collaborative and treatment planning processes.
- Provides crisis assessment and interventions to monitor status of clients in crisis. Provides techniques and behavioral interventions to de-escalate crisis situations or manage behavioral issues.
- Responsible for Maintaining Client and Staff Records According to Agency Policy
- Participates in weekly supervision and meetings to review client and program progress.
- Completes documentation for weekly supervision and consultation provided to case managers.
- Participates in team meetings with case managers.
- Develops and coordinates care with therapeutic providers.
- Coordinates matching efforts between the TFC Supervisor and Recruiter to meet the needs of youth.
- Provides oversite of pre-placement visits for youth to support stable transitions.
- Provides training, clinical consultation, and support to therapeutic foster parents and case managers in monthly support group meetings, in the home and community and in the office as needed.
- Provides master level back up support and coverage for after hours on call therapists sharing monthly on call duties with the TFC Supervisor.
QUALIFICATIONS:
Education: Master’s Degree in mental health field.
Experience: Demonstrated in-depth knowledge and experience as a psychotherapist working with people with mental illness and/or serious emotional disorders. Demonstrated training and experience in all aspects of mental health treatment, assessment, evaluation and provision of individual, group and family therapy. Relevant experience to program model to which individual is assigned.
Licensure/Certification/Registration: LCSW, LMFT, LPC, or Licensed as Clinical Psychologist in State of CT required.
