Qualifications

Current certification as a Certified Nursing Assistant

Certification must remain current.

Good standing with the Nurse Aide Registry

Student Nurse preferred.

No two year break in work history as a C.N.A

Basic Job Responsibility

To provide routine daily nursing care and services that support the care delivered to residents in accordance with established policies, procedures, residents care plans and as directed by a nursing supervisor.

Responsibility

  • Provides care as directed by the licensed nurse to residents requiring long-term, rehabilitative and/or restorative care.
  • Reports observations and other pertinent information related to the care of the resident.
  • Documents objective information related to resident care.
  • Provides services that support that care delivered to the resident.
  • Provides input in the formulation and evaluation of standards of care.
  • Participates in the identification of staff educational needs.
  • Conducts self in a professional manner in compliance with unit/facility policies.
  • Participates in the quality assessment and improvement process.
  • Performs other related duties as assigned or requested.
  • Assures Resident Safety at all times.

Supervision

Receives administrative supervision from the Director of Nursing, the Assistant Director of Nursing or the Nursing Supervisor. May receive functional supervision from registered or licensed nurses working on the unit and/or shift.

Supervision Responsibilities

May exercise functional supervision, responsibility, and accountability necessary to carry out assigned duties.

May serve as a “mentor” to newly hired C.N.A.’s to ensure the standards of care are provided.

May assist with the evaluation of newly hired C.N.A.’s.

Exposure Risk

High risk exposure to blood and body fluids.

Minimum Performance Standards

Performance in the following areas is acceptable when:

  • General Resident Care
  • Residents are identified prior to administering treatments, serving meals, etc. per facility policy.
  • Residents are approached in a kind, gentle, and friendly manner. Respect for the resident’s dignity and privacy is consistently provided. Confidentiality of all resident information is maintained.
  • All grievances and complaints made by the resident and/or their sponsor are reported to the Charge Nurse.
  • Assistance is provided with admissions/transfers/discharges
  • Assist with room clearing after resident transfer/discharge
  • Assist with admissions as directed
  • Obtain and transport specimens to the appropriate area.
  • Resident rooms are neat and orderly.
  • Excess linen and supplies are removed from the room.
  • Care plans and/or resident care cards are reviewed daily to determine if changes in routine care has been made.
  • Assistance with resident care is provided as directed and may include:
  • Baths
  • Linen changes
  • Positioning
  • AM/PM care provided
  • Vital Signs
  • Assistance with restorative programs may include:
  • Positioning and alignment
  • Skin Integrity check/prevention
  • Range of Motion
  • Activities of daily living
  • Bowel & Bladder
  • Dining
  • Ambulation
  • Emotional support

Hallways are kept clear and/or equipment kept to one side of the corridor without blocking fire door and exits.

  • Assist with maintaining the unit in an orderly fashion.
  • Meal trays are distributed and picked up.
  • Assist resident with meals as directed
  • Bed pans, urinals and emesis basins are distributed, picked up and stored properly as needed.
  • Specimens are collected as directed.
  • Assist with treatments as directed.
  • Resident call lights are answered promptly. Appropriate responses to requests are provided.

Supplies & Equipment

  • All equipment is operated in a safe manner and the equipment is not operated without training of Equipment usage has occurred.
  • Defective equipment is reported to the Charge Nurse, Maintenance person and/or designee.
  • Vacant rooms are neat in appearance.
  • Assist with maintenance of the nurse’s station.
  • Equipment is maintained/stored appropriately during rounds (cleaning, storage, refills)
  • Ice containers
  • Geri-chairs
  • Over-bed tables
  • IV poles/tube feeding poles
  • Stretchers/lifts
  • Utility room is monitored routinely for equipment and/or supplies that should be returned to the appropriate department: meal trays, IV poles, linen, etc.
  • Nourishment refrigerator is cleaned according to schedule.
  • Resident room/environment is safe and clean.
  • Resident supplies are restocked as needed.

Facility Procedures

  • Procedures are performed according to established procedures/methods as directed in the policies and procedures manual.
  • Universal precautions and other appropriate form.
  • Intake and output is obtained as directed and recorded accurately on the appropriate form.
  • Resident weights are obtained and recorded as directed.
  • Sign in sheets, flow sheets, etc. is maintained as applicable
  • Documentation of restorative programs and progress is recorded per facility policy.
  • Only use of approved abbreviation is used when recording information.

Communication

  • All changes in a resident’s condition are reported as soon as possible to the Charge Nurse.
  • Staff meetings and in-services are attended.
  • Appropriate procedures are followed when unable to attend meetings.

Professionalism

  • Decisions are made that reflect both knowledge and good judgment.
  • Awareness of limitations is evident and assistance is sought as necessary.
  • Dress code is followed.
  • Work schedule is followed and assigned tasks are completed according to the policies and procedures of the facility.
  • Established procedures are followed when unable to complete according to the policies and procedures of the facility.
  • Organizational ability and time management is demonstrated.
  • Confidentiality of resident information is respected at all times.
  • Cooperate with inter-departmental personnel to ensure that the needs of the residents are met.
  • Disagreements between co-workers, other departments, etc. are handled in a professional manner.
  • Positive working relationships with residents, visitors, and facility staff is demonstrated.
  • Authority is acknowledged and response to the direction of supervisors is appropriate.
  • Time is spent with residents rather than other personnel.
  • Readily assist co-workers as needed.

Working Conditions

  • Works in the nursing services areas, resident rooms, nurse’s station, dining room, etc.
  • Sits, stands, bends, lifts, reaches, walks and moves intermittently during the working hours.
  • Subject to frequent interruptions.
  • Subject to fluctuating noise levels due to phones, mechanical equipment and alarms, construction, residents, etc.
  • Frequent involvement with residents, personnel, visitors, government agency representatives, etc. under all conditions and circumstances.
  • Subject to hostile and emotionally upset residents, personnel, visitors, and family members.
  • Communicates with the staff, nursing personnel, department supervisors, other service providers and vendors.
  • Subject to work beyond normal working hours and may fill other positions temporarily, as necessary.
  • Subject to hazards in the work area and may include: sharp injuries, falls, exposure to odors, burns from equipment, chemicals, etc. throughout work hours.
  • Subject to exposure of infectious waste, disease or conditions.
  • May be required to wear a face mask, gown and/or gloves.

Specific Requirements

  • Must be able to relate information concerning a resident’s condition.
  • Must be willing to work effectively with professional and non-professional personnel.

Physical and Sensory Requirements

  • Able to move throughout the work day.
  • Able to speak the English language in an understandable manner.
  • Able to cope with mental and emotional stress.
  • Able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of the position can be fully met (i.e. hear an alarm, read a blood pressure measurement, etc.)
  • Able to function independently, personal integrity, flexibility and the interpersonal skills to work effectively with residents, personnel, support staff and agencies, the public, etc.
  • In good general health and demonstrate emotional stability.
  • Able to relate and work with the disabled, diseased/ill, elderly, emotionally upset or hostile residents, visitors, staff, etc. within the facility.
  • Able to push, pull, lift and move a minimum of 50 pounds.
  • Able to assist with the evacuation of residents.

Safety Standards

  • Knowledgeable of Quality Assurance and Performance Improvement QAPI program.
  • Must participate in QAPI.
  • Knowledgeable of Person Centered Care.

  • Promotes and Practices safety within the work place.
