Job Description

Coastal Medical is seeking a Nurse Practitioner to join our Hillside Family Medicine Team in Pawtucket. Work in collaboration with a dedicated team of professionals as you provide accurate assessment, management of health problems, health counseling, diagnosis and treatment for our patients ranging from newborn to geriatric.

Coastal Medical is a Patient Centered Medical Home (PCMH) Group Practice. A Patient Centered Medical Home (PCMH) is a primary care medical office that puts the patient in the center of their health care. The PCMH model of care creates care teams that help patients understand all of the services that they will receive today, or may need in the future - even if the services are not being provided in the primary care office.

Duties and Responsibilities

Responsibilities include but are not limited to:

  • Evaluate sick and urgent care patients, transitional care patients and primary care patients
  • Educate patients on maintaining proper health
  • Evaluate, make recommendations, co-manage and treat patients' medical needs
  • Participate in an interdisciplinary team providing care and coordination of our patients ensuring the highest level of care

Job Requirements

  • Active RI Nurse Practitioner (in good standing)
  • Current National Board Certification
  • A minimum of 3 years of primary care experience as a NP in either a family medicine or primary care practice
  • Effective verbal, written, and electronic communication skills
  • Outstanding organizational skills and ability to multi-task
  • Initiative, problem solving ability, adaptability and flexibility
  • Ability to work without direct supervision and practice autonomously
  • Capacity to collaborate with professional colleagues as necessary to provide quality care
  • Must be able to present a friendly, welcoming and professional appearance
