TITLE: Day Rehabilitation Case Manager

MAIN FUNCTION:

This person is responsible for the implementation and delivery of the day rehab center. They perform the roles of instructor, advocate, and case manager for the consumers.

DUTIES AND RESPONSIBILITIES:

• Orienting new consumers and providing them a list of their rights upon enrollment and annually thereafter.
• Responsible for obtaining a copy of a biopsychosocial assessment that has been completed by a licensed professional within 30 days of admission to the program. As appropriate, it may be a substance abuse evaluation.
• Responsible for the development of an individualized treatment plan/rehab plan.
• Assist consumer in comprehending and understanding their diagnosis.
• Responsible for complying with and monitoring the program’s medication provision policy.
• Responsible for complying with the program policy for food service.
• Ability to teach independent living skills through modeling and facilitating learning groups.
• Build rapport with consumers.
• Promote agency awareness through outside resources.
• Ability to assist the consumers in referral to other agencies if needed (i.e. dental, medical, etc.)
• Responsible for planning, coordination, and delivery of day rehab services.
• Assisting consumers in planning and delivery of a variety of evening and/or weekend social activities as needed.
• Transporting consumers when appropriate.
• Perform outreach and/or crisis intervention when needed.
• Public speaking for purpose of public education.
• Complete paperwork as outlined in agency policies and procedures.
• Assists in quality assurance.
• Adheres to all agency policy and procedures.
• Other duties as assigned.

REQUIREMENTS:

To be able to teach appropriate behavior, daily living skills, community living skills, set limits, and provide structure to consumers. To maintain ethics as dictated by profession. Knowledge of counseling theory and practice. Ability to form therapeutic relationships with clients and cooperative, supportive relationships with staff. Ability to maintain flexibility and openness to new information. Skill in oral and written communication. Willing to be a team member with all staff.

TRAINING & EXPERIENCE:

Bachelor’s Degree or higher in human services field.

Job Type: Full-time

Salary: $30,000.00 to $40,000.00 /year
