Job Title: REGISTERED NURSE Supervised by: BRANCH DIRECTOR
Job Summary: Registered Nurse (RN) is responsible for planning, coordinating, providing and documenting care for patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the RN develops implements and follows a plan of care that meets the specific needs of each patient and is in compliance with federal and state regulations, reimbursement guidelines and Ancora policies and procedures.
Duties and Responsibilities:
- Perform comprehensive assessment on all patients as assigned by Director.
- Assess and reassess pain, utilize appropriate pain management techniques and educate the patient and family regarding pain management.
- Assist in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain and notify the primary care physician when pain is not manageable on the current medications.
- Develop a written plan of care for each patient as assigned by Director.
- Provide skilled nursing care.
- Respond to the physical, mental, social and emotional needs of patients and families.
- Assess patient’s condition, initiate, re-evaluate and update care plan as necessary.
- Follow up with obtaining lab results and notify physician of any significant results.
- Perform patient care responsibilities considering needs specific to the standard of care for patient’s age and clinical condition.
- Demonstrate knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.
- Follow the five medication rights and reduce the potential for medication errors.
- Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Collaborate with and obtain orders from physician as patient’s needs dictate.
- Initiate needed referrals to other disciplines and/or community resources.
- Complete and document CNA supervision no less than every fourteen days.
- Initiate case conferences/IDT meetings to discuss multidisciplinary/interdisciplinary team responsibilities, pain management, bereavement counseling, new problems, etc.
- Notify Branch Director of patients who need to be seen on weekends or holidays and submit updated nursing care plan to the weekend/holiday nurse.
- Obtain needed supplies and equipment to provide care.
- Address and support cultural practices, as long as such practices do not harm others.
- Adhere to all Ancora payroll, billing and documentation policies and procedures.
- Knowledge of current federal, state, and Ancora regulations concerning patient care.
Professional Requirements:
- Adhere to dress code, appearance is neat and clean.
- Complete annual education requirements.
- Maintain regulatory requirements.
- Maintain patient confidentiality at all times.
- Report to work on time and as scheduled, complete work within designated time.
- Wear identification while on duty.
- Attend annual review and inservices.
- Attend staff meetings as scheduled and complete related educational activities.
- Comply with all Ancora policies regarding ethical business practices.
- Actively participate in performance improvement and continuous quality improvement activities such as, inservices, staff meetings, Professional Advisory Committee, etc.
- Represent Ancora in a positive and professional manner in the community.
- Communicate the mission, ethics and goals of Ancora.
General Requirements:
- Graduate of an accredited school of nursing.
- Current Registered Nurse licensure in the State of Alaska.
- Previous experience as an RN in home health, hospice, geriatrics or general medical/surgical nursing.
- Current BCLS certification.
- Current health certificate/physical examination and TB testing.
- Valid state driver’s license and reliable automobile.
- Proof of current automobile insurance.
- Ability to relate well to staff and other professionals.
Language Skills:
- Ability to effectively communicate in English, both verbally and in writing.
- Additional languages preferred.
General Skills:
- Basic computer knowledge.
- Comprehensive knowledge of general nursing theory and practice.
Physical Demands:
- Mobility, reaching, bending, lifting, talking, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, ability to communicate verbally and the ability to remain calm under stress.
