C/A In-Home Team Case Manager Job Opening

C/A In-Home Team Case Manager (Covington County) Employees in this position are expected to be flexible, professional and a team player. Individuals will work as a team with a MS therapist and schedule home visits, obtain relevant information from children and their families, write plans of care, establish liaisons with other service agencies in the community, and assist consumers in obtaining the services they are entitled to receive. Must have Bachelor's Degree in a human services related field, case management trained preferred; currentAL driver's license; eligibility for coverageunder center liability insurance and individually held vehicle liability insurance and clear background check.
