Support Staff are Responsible for ensuring the individual’s safety and well-being as well as upholding their dignity, self-worth, and choice. Support Staff are expected to respect an individual’s personal preference, personality, priorities and personal space.

Support Staff are to keep foremost in their mind that it is the individual’s home and that they are employed to support the individual. Support Staff’s attention and focus should always be upon the person who is being supported.

The Individualized Support Plan provides the information that is needed regarding how to encourage and support the individual in the areas listed above. It is the Support Staff’s responsibility to know and follow the Individualized Support Plan.

SUPPORT STAFF ESSENTIAL JOB DUTIES:

  • Provide guidance, instruction, training, and observation to individuals in the areas of self-care, academics, housekeeping, laundry skills, meal preparation, social/leisure activities, and community participation as outlined in the Individualized Support Plan.
  • Provide accurate data collection and record keeping of services listed above.
  • Manage/modify individual’s maladaptive behavior by working with the individual and providing intervention as outlined in the Individualized Support Plan and/or Behavior Support Plan.
  • Serve as a positive role model for individuals to promote appropriate, adult behaviors.
  • Uphold individual’s civil and human rights.
  • Treat individual(s) with dignity and respect.
  • Provide protective oversight at the residential setting and in the community.
  • Upon completion of the Level I Medication Aide course, administer medications and treatments as prescribed and to maintain accurate documentation of same.
  • Accompany individuals to appointments, obtaining written documentation of services provided. (Note: This duty is required when delegated by the Team Leader and PM.)
  • Assist individual(s) in planning and engaging in age appropriate social and recreational activities both in the residential setting and in the community.
  • Encourage/assist individual(s) in planning and preparing healthy meals and snacks, and to follow recommendations of his/her health providers.
  • Observe and document individual’s daily activities, behaviors, medical issues, etc.
  • Complete any event report before leaving shift of duty.
  • Complete a staff note (Daily Progress Note) detailing progress on outcomes, supports provided, food intake, etc. before leaving shift of duty.
  • Be knowledgeable of fire safety/emergency procedures and assist individual(s) with evacuation when necessary.
  • Interact politely with consumer’s parents/guardians, family members/friends, Support Coordinator, TJ Swift House, Inc. staff, etc.
  • Perform housekeeping/maintenance tasks as assigned to provide a clean, safe, healthy environment for individuals served.

PHYSICAL/MENTAL DEMANDS:

  • May require some lifting (not to exceed 100 pounds)
  • Combination of standing, sitting, and/or walking for long periods of time, reaching, bending and/or stooping, and traveling (driving or riding) long distances
  • Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus
  • Occasional stress from balancing multiple tasks

EDUCATION/EXPERIENCE/KNOWLEDGE:

  • High School diploma or GED

  • Insurance is offered to employee's after 90 days and your are working at least 32 hours or more.
  • one week vacation and two personal days are available after one year of employment and staff are working at least 32 hours or more.
