Physical Therapist - FT PTN Job Opening

Millenium Home Health, a trusted provider of home health care, is seeking full-time and PRN Physical Therapists to care for patients in Seaford and the surrounding area.

Job Responsibilities:

In this role, you'll provide physical therapy services to patients in their home as prescribed by the attending physician; recording clinical and progress notes as well as coordinating other patient activities/services to ensure the highest quality of care.

Requirements:

* Current Delaware Physical Therapy license

* Current CPR certification

* 1 yr of clinical experience preferably in home care

* Demonstrated knowledge in the assessment, implementation and evaluation of client response to actual/potential medical problems

* Excellent communication, organization and interpersonal skills

* Reliable transportation
