Social Worker
GENERAL PURPOSE:
The social worker is responsible for the implementation of standards of care for medical social work services. The social worker assesses the psychosocial and economic needs of patients and develop treatment plans to meet those needs in accordance with government regulations and Company policies to assist staff in implementing the patient’s plan of care.
QUALIFICATIONS:
Must be a licensed social worker in the state employed or otherwise be able to meet state and federal regulations.
Must possess a minimum of a Bachelor’s Degree in Social Work with Master’s Degree preferred. Bachelor’s candidates must adhere to supervisory requirements.
Minimum of one (1) year experience in health care, preferably Hospice or Home Health.
Completion of post-secondary education and be able to read, write and follow oral and written directions.
Must be able to relate positively and favorably to patients, families and outside agencies and to work cooperatively with other Employees.
Must be capable of maintaining regularly scheduled attendance.
Demonstrates good verbal and written communication, and organization skills.
Possesses and maintains current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Must be capable of performing the job functions of this position with or without accommodations.
ESSENTIAL JOB FUNCTIONS:
Management Function:
Plan, develop, organize, implement, evaluate, supervise and direct the psychosocial and economic needs of the patient.
Carries out social evaluations and plans intervention based on evaluation findings.
Maintains clinical records on all patients referred to social work.
Provides information and referral services for Company patients and families/caregivers regarding practical and environmental needs.
Provides information to patients or families/caregivers and community agencies.
Serves as liaison between patients or families/caregivers and community agencies.
Maintains collaborative relationships with Company personnel to support patient care.
Maintains and develops contracts with public and private agencies as resources for patient and Company personnel.
Assists physician and other team members in understanding significant social and emotional factors related to health problems.
Care Planning Assessment Function:
Participates in the development of the total plan of care and case conferences as required.
Participate in assessing, reviewing and revising care plans as required.
Participates in discharge planning.
Teamwork Function:
Coordinate services to assure a smooth integration of interdisciplinary approach to quality patient care.
Patient Rights Function:
Maintain patient confidentiality.
Understand and comply with patient rights regulations.
Regulatory Compliance Function:
Maintain a working knowledge of government regulations and ensure compliance.
Education Function:
Conduct and participate in agency education activities, attend and participate in staff meetings.
Maintain a professional competence through participation in continuing education programs, seminars, and training programs.
Other Functions:
Adheres to all of the Company’s policies and procedures.
Perform other duties as assigned.
